Designing forms seems to be a lost art. I started
producing forms with pen and ink and an Xacto knife
in 1967 at my first job in Montreal. Today they are all
done electronically. With Adobe Acrobat Reader as a
free download, the forms can be filled in on the
computer and automatically emailed back to
wherever the information is required. They can be as
simple as a check box or they can be produced with
drop down lists, radio buttons, multiple choice
answers, etc. for gathering whatever information is
needed. It does require a little programming along
with the design.
